Yes.  Currently we accept Medicare, Health Partners, UCare, some MN BCBS plans, TriWest, some insurance plan out-of-network benefits and have private pay options. 

Yes, you will need a referral to work with us.  Your primary care provider, optometrist or ophthalmologist can provide you with a referral.  Please see our FORMS section above to find a copy of the referral forms.

• Once we have received your referral we can schedule you for an initial occupational therapy evaluation.
• The initial evaluation can be up to 90 minutes long.
• During the evaluation, we will assess your functional abilities, areas of challenge, identify your goals and determine an individualized plan of care to help you reach your goals.
• Typically we will see patients 1 time/week.
• Follow-up visits will be scheduled with length of visit to be determined.

We currently  serve the Twin Cities West Metro and surrounding areas- a 25 mile radius from Waconia, MN.
